I look at the patient’s medication charts on the acute wards, frustrated that vulnerable patients have been prescribed multiple medication charts. The charts are like winter shades of leaves sometimes taped with cello tape!  The medications could be written on one medication chart. This would ensure the medication is precise and make the demanding job of the nurses easy. Of course, prevent errors.  Most time I have to re-write the medication charts as the senior clinicians on the acute ward despite my other commitments.  Don’t get me wrong my colleagues were fantastic and well-intentioned people but the system is at a breaking point!

We have discussed this during the team meetings, but because of the busy wards this is a recurrent problem and no one is taking leadership. The audits are great but much like many other solutions that are proposed to the climate crisis right now are sticking plasters.

According to sex therapist, couples counselor, and relationship coach Lyndsey Murray, they absolutely can. The relationship Matters Therapy founder says that optimist and pessimists can offer new, valuable perspectives to their partners and help each other well-informed decisions.
